Non-critical: Observed non-commercial appliances in use in the kitchen, gasket on 4 door pull-out drawer in disrepair, observed build-up and food debris on floor of walk-in freezer and along equipment on main line, the counter in the bar area was repaired with duct tape and debris and floor build-up behind prep refrigerator, under three bay sink, floor area near downstairs meat slicer, dry storage room downstairs, utility room where mop sink is located and employee bath room.

Critical: Time stickers were missing on non-refrigerated lettuce on prep line. Violation corrected at time of inspection.

Non-critical: Observed ice build-up on boxes of food in walk-in freezer and build-up of residue under self-serve pop machine.

Super Valu, Archbold

Critical: Observed banana chips, sugar-free candies and other bulk candy with no list of ingredients and some deli meats were out of date. Discussed the sequence of time dating food items, removed offending items and violation was corrected.

Non-critical: Observed debris and residue under shelves in walk-in cooler, around sink in deli area and under shelves in dry storage area, still cracked / missing floor tiles to replace, compressor in walk-in cooler in meat department is leaking and observed non-commercial appliances in use in the kitchen.

No violations observed in Archbold: Casa Vieja, Fairlawn Haven and K of C Rebeau Hall.

Critical: Ingredient list for donuts and other baked goods were not in plain view and white rice leftover from previous day is not properly tested for desired pH levels. Disposed rice at time of inspection.

Critical: Cold food holding temperatures were too warm and some ingredient lists on bakery items were insufficient.

Non-critical: Observed food items stored in single-use storage containers, nonfood contact surfaces were dirty and non-commercial appliances in use in the kitchen.

Fulton County Health Center, Wauseon

Critical: Observed several food items not date marked properly, items like salad, chicken salad, pumpkin pie were not labeled with ingredients and no thermometer in juice dispensing unit.

Little Caesars, Wauseon

Non-critical: Plumbing beneath three compartment sink was leaking.

Los Mariachis Mexican Restaurant, Wauseon

Critical: Observed cutting edge of can opener with food build-up and reside build-up on pop nozzles plus chicken, shrimp and other meats were not at optimum cold holding temperatures.

Non-critical: Observed non-commercial appliances in use in the kitchen, pull-out drawer fridge was holding poor temperatures and build-up and debris on water line outside of ice machine.

McDonald’s, Wauseon

Critical: Food in small prep fridge on main line was at 50 degrees – should be at 41 degrees. Food items were pulled and violation corrected at time of inspection. Manager will call after repairs have been made.

Non-critical: Observed food debris around equipment.

Murphy’s Pub, Wauseon

Non-critical: Observed reach-in cooler was in disrepair, areas of floor damaged and no hand washing signage present.

Ryan’s Restaurant, Wauseon

Critical: At time of inspection, walk-in cooler had an ambient temperature of 48 degrees and internal temperature which ranged between 46 and 47 degrees and the meat slicer was not broken down and cleaned since the weekend. Upon follow-up visit, all critical violations have been remedied and items involved in non-critical items are being addressed.

Non-critical: Observed non-commercial appliances in use in the kitchen, water and food debris build up in small prep cooler, build-up and residue under racks in dry storage area and on racks under dish washing room. and light shields were missing in walk-in cooler.

Sullivans Restaurant, Wauseon

Critical: Small prep cooler towards east side of kitchen had an ambient temperature of 50 degrees and internal temperature 49 degrees at time of inspection.

Non-critical: Some containers held in dry storage had no labels, an icy build-up in walk-in freezer was dripping water onto food items, cutting boards had deep cuts, a non-commercial appliance in use in the kitchen, there was no air gap between two bay sink and small prep refrigerator had gaskets in disrepair (gaskets were on order). Manager is to call Board of Health when unit is repaired.

Tiffany’s, Wauseon

Critical: Observed potato slicer had not been broken down and properly cleaned.

Non-critical: Observed non-commercial appliances in use in the kitchen, freezer units had icy build-up with food items in contact with ice, ceiling paint was chipped and build-up of food and debris under two bay sink, around fryer area, shelves in waitress area, around ice machine and under kitchen equipment.

Non-critical: Cove base in walk-in cooler is pulling away from the wall.
